The University of Maryland, Baltimore School of Nursing is seeking a Community Outreach Worker I (Contractual). Under direct supervision, the Community Outreach Worker I will provide outreach and administrative support for the Governor's Wellmobile clinic. The mission of the Governor's Wellmobile is to provide primary and preventive health care services to geographically underserved communities and uninsured individuals across the state and to serve as a principle training site for expanding University of Maryland, Baltimore School of Nursing student learning opportunities in caring for underserved populations.

Essential Functions:

  • Contact clients in person, by phone, or writing to ensure they have completed required or recommended actions. Communicate to patients their scheduled appointments with the Wellmobile and external clinical providers and what they need to bring to the appointments.
  • Provides general information to individuals and families on program objectives and services; eligibility requirements and benefits; confidentiality of information, etc. Distributes informational materials and literature.
  • Interpret, translate (Spanish-English), or provide cultural mediation related to health services or information for current and future Wellmobile clients.
  • Schedules clients for appointments with health care providers, reminds them of pending appointments, and contacts them to inquire into reasons for missed appointments.
  • Assist the receptionist and nurse case manager with patient registration and intake process. Completes related documentation.
  • Assist with client retention by following up on all contracts for continued progress assessment and locating those clients who have moved or lost contact with the program.
  • Participate in staff meetings and conferences. Reports on patient contacts and delegated follow-up activities.
  • Compile medical records weekly and as needed for patient care visits based on daily client schedule.
  • Prepare written reports and maintain records of outreach contacts and activities.
  • Assist with exam room set-up and clean-up and daily maintenance of the clinical areas.
  • Maintain a good working relationship with all members of the community.

Qualifications

Education: High School Diploma or GED

Preferred Education: Bachelor's degree is preferred

Preferred Experience: At least one year of health care outreach worker experience in an underserved community preferred. Experience with underserved populations preferred.

Required Knowledge/ Skills/ Abilities:

Ability to communicate effectively orally and in writing . Ability to establish and maintain effective working relationships. Ability to operate computers and other office equipment. Ability to write reports and maintain records. Ability to work in extreme weather conditions. Ability to perform extensive standing and walki ng . Ability to handle sensitive and confidential matters with discretion and tact. Ability to interact with the public, health care providers, community organizations, and social service agencies. Ability to maintain poise and courtesy under pressure. Knowledge of principles and processes for providing customer and personal services; this includes customer needs assessment, meeting quality standards for services, an evaluation of customer satisfaction.

Proficiency in written and spoken medical Spanish and medical interpretation Spanish- English and English to Spanish preferred .

Except for qualifications established by law, additional related experience and formal education in which one has gained the knowledge, skills, and abilities required for full performance of the work of the job class may be substituted for the education or experience requirement on a year-for - year basis with 30 college credits being equivalent to one year of experience.

CONDITIONS OF EMPLOYMENT

Employees in this job class may be required to use their personal automobile while conducting official business and possess a valid Maryland Non-commercial Class C or Commercial Class B Driver's License (CDL). Candidates selected for employment may be subjected to medical inquiries and/or medical examination to determine ability to perform the job. Employees in this job class may be required to successfully pass a police background check.

Rate of Pay: $19. 33 / hour
